Welcome to the eco-friendly Point Dume new student enrollment process

For student enrollment, please complete the enrollment packet and bring the following ORIGINAL DOCUMENTS to the school office:

Child’s Certified Birth Certificate
Child’s immunization records - Click Here for State of CA Immunization Requirements for School Entry
Parent Photo Identification – current driver’s license or Passport
Proof of Residency – Click here for Residency requirements.
Upon verification, your original documents will be returned to you. (Faxes and photocopies are not accepted.)

Student Emergency and Information Card
Student Emergency and Information Card information must be complete, accurate, and kept updated throughout the school year. Parents are always notified first in the event of illness or emergency. In the event the parents cannot bereached, please designate at least three (3) LOCAL contact persons .It is best to designate persons who live or work within Malibu. If your child must leave school before dismissal time for any reason, your child can be released only to persons named on the emergency card.

Department of Health – This form is not available online. Please pick up one in the school office. It is to be completed by a parent and does NOT require a doctor’s signature. Use your child’s medical records to complete this form.

KINDERGARTEN ONLY - In addition to the forms listed above, please print the following items for incoming kindergartners.
Kindergarten Information Sheet
Oral Health Assessment - Section 2 is completed and signed by your child’s dentist. It is required by the end of child’s first year in a public school.
Report of Health Examination - Part II is completed and signed by your child’s physician. A health screening examination is required for all children entering the 1st grade. Incoming kindergartners MAY choose to fulfill this requirement at the time of Kindergarten enrollment after all of the immunizations are completed.
